| Health History | This form needs to be completed by the new patient for Dr. Farrukh, Dr. Tomczyk, and Dr. Romaniuk only. |
| Pre-Visit Questionnaire | This form needs to be completed by the new patient for Dr. Sheikh only. |
| Patient Information | This form needs to be completed by all new patients for all doctors. |
| Pediatric Screening | This form needs to be completed by all patients younger than 18 and have an appointment with Dr. Tomczyk or Dr. Romaniuk only. *Please bring copy of shot record with patient. Shot records can be obtained from the school the patient is attending or from the Health Department. |
